Legal: Death in Custody/Self Inflicted Death Forms

There appears to be some confusion following the promulgation of the new Legal Advice Handbook in respect of the above.

The old LA490 Death in Custody form has been replaced with Form 6 Self Inflicted Deaths. This form must be used to report all deaths in custody to ensure our legal advisors are aware of the death and register as an interested party.

We acknowledge that a number of deaths may not be self-inflicted, but it is vital that we are made aware of all deaths in custody, in case a conflict arises in the future.  As you know, the Prisons and Probation Ombudsman investigates all deaths and as a result, members may be required to attend the Coroners Court at some point in the future.
